一種主要的醫院病原體，艱難梭菌（Clostridium difficile）引起抗生素相關性結腸炎和腸道炎癥性疾病，一起稱為CDI。
C艱難梭菌毒素如何引起腹瀉？ 通過釋放兩種大蛋白腸毒素，艱難梭菌毒素A（TcdA）和艱難梭菌毒素B（TcdB），艱難梭菌介導炎性腹瀉并損害腸??上皮細胞。施用于培養細胞時的毒素都會破壞細胞骨架并引發細胞凋亡依賴性細胞凋亡，盡管艱難梭菌毒素B的效力比艱難梭菌多很多倍。毒素A.兩種毒素還激活腸上皮細胞和免疫細胞以產生細胞因子和趨化因子，可能導致與嚴重CDI相關的并發癥。TcdA和TcdB的細胞內靶標是RhoGTP酶，其調節許多宿主細胞過程，包括建立上皮屏障和免疫細胞的遷移和信號傳導。當Rho蛋白被艱難梭菌毒素A或B 糖基化時，信號被破壞，導致CDI的腸損傷和炎癥特征。
